How to Complete the NSFAS Consent Form

Use black ink, capital letters, and clear handwriting. Don’t use correction fluid; initial any changes. No electronic signatures allowed. Here’s what to include:

Your Details:

    • ID Number

    • Initials & Surname (like on your ID)

    • Email Address

    • Cellphone Number

    • Signature & Date (YYYY MM DD)

Parent/Guardian Details (check the right boxes):

    • Father’s/Guardian’s Initials & Surname

    • ID Number

    • Email & Cellphone

    • Signature & Date

  • Mother/Guardian Details (if needed):

    • Same as Parent/Guardian Details and only whn needed.

Spouse Details (if married):

    • Initials & Surname

    • ID Number

    • Email & Cellphone

    • Signature & Date
